- Printed request form (the GDT calls it a "learning renewal request form")
- Two photos with a blue background, size 4x6
- Copy of your Civil ID
- Copy of your passport (non-Kuwaitis)
- Blood type and medical examination
- Work permit and a copy (foreigners)
